This will save you time so that you will not have to be at your appointment early to complete these forms.
If you are unable to complete them beforehand please arrive 15-20 minutes before you schedule initial appointment time to complete them in our office.

Acupuncture and Chinese medicine are cumulative therapies. Unless you have a new or acute issue arise, you will need to consistently move through your prescribed treatment plan, series of treatments close together, to gain relief, feel better and maintain improvement of your condition.
Because of the dynamic of this therapeutic healing process Packages are available and encouraged.
Making this commitment to yourself and your health will allow you to book your prescribed sessions in a way that provides the most efficient and best results possible.

Some insurance policies cover acupuncture for certain conditions and, although we don’t process insurance in our office, we are able to provide you with a receipt that has the correct billing codes on it.
This will help you to be reimbursed for your treatment.
